Positioning Clinical Pharmacists’ roles in Indian Health care

Clinical Pharmacist

Clinical pharmacy has emerged as one of the latest branches of pharmacy in the 21st Century. In India, the health care services entailed the team of physicians, nurses and medical laboratory technicians in hospitals. In USA and Canada the effective health care service involves Clinical pharmacists in the team. The main reason to have them in team  is that Clinical pharmacist have versatile knowledge about medication uses; curative and side effect. It is where pharmacists deal with various aspects of patient care, dispensing of drugs and advising patients on the safe and rational use of drugs.

 

Clinical pharmacy is in the growing stage in developing countries, especially in densely populated country like India wherein there is a limited number of hospitals that have clinical pharmacist in decision making to patients.  

 

Clinical pharmacy services are not limited to certain areas but applies to all medical fields such as cardiology, infectious diseases, ambulatory care, oncology, nephrology, internal medicine, …… It also covers in-patient and out-patient services.

 

 

Roles of Clinical Pharmacist in Hospitals:

1.Drug related:

  • If the drug is not yet in hospital formulary: Clinical Pharmacist are responsible for drug evaluation and presentation to the P&T Committee.

  • Before prescribing the medications: Clinical pharmacists provide information about institution formulary and availability, alternatives, drug information, drug-related policies (e.g. if the drug is restricted, investigational).

  • During prescribing the medications:  Clinical pharmacists provide services for prescription monitoring such as reviewing the prescribed drugs including proper drug choice for a specific indication, dosage, frequency, duration, if there is a need for dose adjustment, suggest the proper drug for untreated indication, consider the history of drug allergy, evaluate and manage adverse drug reactions, and possible drug interactions.

  • After prescribing the medications: Clinical pharmacists design a pharmaceutical care plan, identify problems, establish outcome goals, provide patient counseling, monitor pharmacokinetics and therapeutic drug level, report adverse drug reaction (ADRs), evaluate treatment outcome, and drug information to health care professionals.

2.Patient-related: Clinical pharmacists are responsible for:

  • Collecting patient history
  • Patient Education on drug,Collecting and lifestyle modification 
  • Medication reconciliation
  • Provide educational materials
  • Patient follow up
  • Drug Information and Poison Information
  • ADR Assessment
  • Evaluation of drug related problems such as Drug-Drug Interaction, side effects, Drug -Food Interaction, Drug-Herb Interaction and etc
  • Nursing station audit
  • Medication Chart review

3. Other services:

  • Pharmacoeconomic
  • Pharmacovigilance
  • Drug use evaluation (DUE)
  • Specific clinics services as diabetes, hypercholesterolemia clinics, and   anticoagulant clinic
  •  Research: Clinical Research coordinator, clinical Data Management, Medical writing etc.
  • Regulatory affairs
  • Pharmacogenomics
